Recognised advanced standing

Care is led by Steven Davis MACP, APA Titled Gerontological Physiotherapist — formal recognition by the Australian College of Physiotherapists of a highly developed level of skill, clinical reasoning and evidence-based practice in older-adult physiotherapy.

Measured outcomes

Assessments use validated outcome measures with structured scoring, so reviews, care plans and program reporting are backed by real data — not impressions.

Reliable reporting

Timely, useful reports to GPs, case managers and provider teams — including GP care-plan start and end reporting, and DVA treatment-cycle paperwork handled correctly.

Home-based, person-centred

Clients are seen where their goals live. Programs are built around function in their own home and community, with families and carers involved.

Pathways

How to refer

GPs — care plans & DVA

Refer under a chronic condition care plan (Medicare) or DVA treatment cycle (D904 or via your practice software). We provide start and end-of-cycle reporting and bill DVA/Medicare directly where applicable.

Support at Home & CHSP providers

We subcontract physiotherapy for ongoing classifications, the Restorative Care Pathway (up to 16 weeks) and CHSP services — with clear service agreements, insurance and clinical governance documentation available on request.

Residential aged care

Assessment and therapy for residents, plus 30-minute team in-services for Newcastle, Lake Macquarie and Hunter facilities — on recognising pain in dementia and understanding distress as communication and unmet need.

Hospital & discharge teams

Home-based rehabilitation after admission or surgery — a practical option for patients heading home who need goal-focused follow-up where function actually happens.
