Ongoing Services
We partner with Support at Home providers to deliver essential Physiotherapy services under the new classification and quarterly budget model. As a qualified subcontractor, we ensure your clients receive compliant, high-quality clinical care focused on pain management, mobility maintenance, falls prevention, and prescribed exercise programs. We seamlessly integrate with your care management team to meet the strengthened clinical quality standards and reporting needs for ongoing client well-being.
Restorative Care Pathway
Specialising in reablement, we are the dedicated Physiotherapy subcontractor for your intensive, goal-oriented short-term support needs under the new Restorative Care Pathway. Our focus is on delivering high-impact, time-limited intervention (up to 16 weeks) to maximise client function, restore independence post-hospitalisation or setback, and effectively delay the need for higher-level, ongoing support. Partner with us for expert clinical outcomes and timely discharge planning.
Wellness and Reablement
We provide targeted Physiotherapy services to CHSP providers, aligning with the program's core principles of wellness and reablement. As a subcontractor, we focus on delivering effective, episodic interventions like group exercise, mobility assessments, and falls prevention education to keep entry-level clients active and independent for longer. We deliver these services reliably and compliantly under the current block-funded grant model.
